Anyone know the widest fitting shoe available?

I use VivoBarefoot, which are wide, but would like to try something else.

New Balance are too narrow - anyone know a good brand for width in the toe box?

Vibram Five Fingers.

If you are not at all into toe shoes, then Leming footwear has ample room for toe splay.  I have a couple of pair from back when they were Stem footwear and I think they are great.
