"Real" dress shoes, the shoemaker made type is quite ok on your feet actually. The bad thing is the heel (keep it low!), but the leather sole is very firm (nearly no cushioning), and there's no arch support. So after moccasin type of shoes, I think that truly classic gentlemens shoes are the shoes that mess the least with your feet, if you buy a pair that fits properly (which not everyone does).

However, nowadays there's a lot of massproduced "dress shoes" with different kinds of cushioning rubber midsoles and arch support etc, which apart from not looking as good as the classics also mess with your feet.
